Irreducible distal radial fracture due to extensor pollicis longus tendon interposition: a case report

چکیده
Various causes have been reported for irreducible fractures of the distal radius. Most of them have been reported in children. We report a case of irreducible distal radial fracture in an 18-year-old Caucasian male patient. The fracture was not reducible by closed methods and extensor pollicis tendon was found to be interposed during surgical intervention. At one year follow-up, the patient had good radiological evidence of fracture healing and a good functional outcome. This case report highlights the need for a high index of suspicion to rule out soft tissue interposition in cases of irreducible distal radial fractures and a low threshold for open reduction.

Palmaris Longus Tendon Grafting for Extensor Pollicis Longus Tendon Rupture bScrew Tip after 20 years
INTRODUCTION Spontaneous ruptures of extension pollicis longus tendon predominantly occur after undisplaced or minimally displaced distal radial fracture near Lister tubercle. Systemic inflammatory diseases and systemic or local steroid, mechanical causes like bony ridges, presence of bone plate or external fixator pin may precipitate this. Repetitive uses in certain occupation like cooking, co...
متن کاملMuscle moment arms in the first dorsal extensor compartment after radial malunion. A cadaver study.
BACKGROUND Functional loss is a common complication of the fractured distal part of the radius. The purpose of the present study was to determine if the moment arms of the first dorsal extensor compartment are altered by distal radial fracture malunion. We hypothesized that the moment arms of the abductor pollicis longus and extensor pollicis brevis are significantly affected by dorsal angulati...
متن کاملPseudo-rupture of Extensor Pollicis Longus following Kischner wire fixation of distal radius fractures.
We report two cases of extensor pollicis longus tendon tethering following K-wire insertion to treat unstable distal radius fractures, an unusual complication. In both cases, extensor pollicis longus function returned with conservative management. The structures considered "at risk" during K-wire insertion are described, and the common complications are discussed. Operative approaches to minimi...
متن کاملThe Prodrome of Extensor Pollicis Longus Tendonitis and Rupture: Rupture May Be Preventable.
Current literature suggests that tendonitis of the extensor pollicis longus (EPL) is a rare condition that has a high rate of progression to rupture. This study documents the prodrome of impending EPL rupture in patients with prior nondisplaced distal radial fracture. A retrospective study identified patients with EPL tendonitis or tendon rupture. Seven patients (6 females and 1 male) had susta...
متن کاملAnatomic Landmarks for the First Dorsal Compartment
OBJECTIVE Knowledge of anatomic landmarks for the first dorsal compartment can assist clinicians with management of de Quervain's disease. The radial styloid, the scaphoid tubercle, and Lister's tubercle can be used as superficial landmarks for the first dorsal compartment. METHODS Thirty-two cadaveric wrists were dissected, and measurements were taken from the predetermined landmarks to the ...
